Care Coordination Services at Guthrie County Hospital provides services to patients, their families and members of the community which enable them to make the best use of health care resources.
Some examples of services provided include but are not limited to:

- advanced care planning,
- assistance with Medicaid resources,
- referrals for nursing and assisted living facilities as well as hospice, contact with DHS, as well as a variety of other services.
The department works to reduce social, financial, and medical barriers in order to optimize patient and family functioning. Information and referrals provided by the care coordinator are free of charge to all patients, family members and the community.
